Colacurcio Wellness LLC (DBA: Vitality Modern Wellness) is committed to providing a safe and welcoming environment for all members and staff. This Code of Conduct outlines our expectations for behavior within our facility. By becoming a member or working at Colacurcio Wellness LLC (DBA: Vitality Modern Wellness), you agree to abide by these rules.
General Rules: Initial:______
- Respect: Treat all members, staff, and guests with respect and courtesy. Discrimination, harassment, or disrespectful behavior will not be tolerated.
- Safety: Prioritize the safety of yourself and others. Follow all safety guidelines, equipment usage instructions, and report any hazards or accidents immediately.
- Cleanliness: Maintain personal hygiene and clean up after yourself. Return all equipment, dumbbells, and benches to their designated place after use.
- Attire: Always wear appropriate workout attire. Shirts, closed-toe athletic shoes, and other specified attire are required for entry. Socks to be worn with your closed-toe athletic shoes is important as this will reduce the likelihood you will develop blisters or other related foot lesions from repetitive exercise. Socks also help eliminate foot odors and athlete foot type conditions which will aid in our facility operating in a more clean, odor free, and sanitary fashion.
Membership and Guest Policies: Initial:______
- Membership Virtual Key Card: Always have your virtual key card present with your smart phone as this will allow you to unlock the front door to enter. At times we may only have one staff member on site, so the front door will always be locked to ensure the safety of our members, staff, and equipment. If you show up to enter without your virtual key card, you can call the general manager on their cell phone where you may be able to be buzzed in, but this is not a guarantee.
- If you notice a person knocking at the front door, DO NOT waste your time approaching the door and DO NOT let the stranger or solicitor in. If you recognize that it’s a fellow member at the door who may not have their smart phone / virtual key card on them, you may open the door for them so they can gain access.
- Guests must be prescheduled and paid in advance prior to beginning their guest workout/visit or paid for upon the time of their arrival if previously arranged with managment. Guests must follow all club rules. Members are responsible for the conduct of their guests.
- a) Guest fee is $25 per day. Please be aware that there is a restriction on the number of times a specific individual can visit our health club as a guest.
Equipment Usage: Initial:______
- Equipment Courtesy: Limit your time on machines during busy periods and allow others to work in between sets.
- a) Do not get lost in your phone while sitting on and taking up a machine instead of using it. This prevents another member from jumping in and doing a set while you are between sets. This is a common problem at gyms that cater to a younger clientele.
- b) Use a complimentary towel during your workout. This lessens sweat marks left on equipment, tables, machines, etc. Please wipe down noticeable sweat marks, sweat drips, etc using any of the supplied disinfecting cleaners and rags/paper towels, or disinfecting wipes provided, etc.
- Proper Use: Use equipment as intended. Most equipment has directions and or diagrams with instructions on how to use, affixed on the actual piece of equipment. Ask staff for guidance if you are unsure how to use a machine.
Personal Conduct: Initial:______
- Noise Level: Keep noise to a normal level. Use headphones, ear buds, ear pods, etc. for music, audio books, podcasts, etc. DO NOT USE SPEAKER PHONE TO TALK OR TO LISTEN TO ANYTHING ON YOUR SMART PHONE, ETC.
- DISTRACTING MEMBERS: Member are here to exercise and many DO NOT want to be interrupted. Asking a member if you can jump in on a machine or piece of equipment between their set, or to ask if they are using or still using a machine, etc is NORMAL GYM ETIQUETTE. But interrupting a member’s workout to make small talk can be distracting and annoying especially since most people are in a hurry to get their workout in and to get on with their day. Please use SELF AWARENESS ON THIS…IF A MEMBER HAS HEADPHONES ON AND IS AMIDST A WORKOUT AND THEY ARE NOT MAKING EYE CONTACT WITH YOU THAT’S MOST LIKELY CAUSE THEY ARE IN A HURRY, ETC, AND DO NOT WANT TO BE INTERUPTED, SO TAKE THIS AS A QUE NOT TO INTERRUPT UNTIL LATER OR PERHAPS BETWEEN THEIR SET. IF IT IS ABSOLUTELY NECESSARY, OR IF YOU DO HAVE TO INTERUPT THIS PERSON TO SAY SOMETHING, JUST KEEP IT BRIEF SO THEY CAN HEAR YOU, HAVE A CHANCE TO RESPOND, AND THEN THEY CAN GET RIGHT BACK TO THEIR WORKOUT. This scenario happens a lot at health clubs, many years ago when I worked at NYSC West Caldwell, one of the trainers there used to wear headphones when working out just so less people would come up to him and interrupt his workout.
- Personal Belongings: Store personal belongings in provided lockers. Colacurcio Wellness LLC (DBA: Vitality Modern Wellness) is not responsible for lost or stolen items.
Lockers: Initial:______
- Lockers are for single day use. You may not leave your belongings locked up in a locker overnight.
- Any changing of shirts, pants, etc must be done in your respective rest room.
- All belongings such as shoes, pocket books, back packs, etc must be stored inside a locker so clutter does not build up in front of the lockers.
Sauna(s): Initial:______
- Shoes MUST be removed before entering a sauna.
- Time in a sauna is limited to 30 minutes.
- You must sign up to use a sauna. There will be a daily sign-up sheet for each sauna which will be located outside of the sauna.
Health and Wellness: Initial:______
- Health Conditions: If you have any medical conditions or concerns, consult a healthcare professional before beginning an exercise program.

Kitchen Rules: Initial:______
- Refrigerator / Freezer are accessible to members. Feel free to store items in either. Expired items will be thrown out appropriately to maintain a healthy environment.
- DO NOT eat / use / or take any items from the Refrigerator / Freezer that do not belong to you.
- If you use a blender, or any dishes, cups, silverware, etc, you MUST WASH these items when finished.
Enforcement: Initial:______
- Violations: Failure to comply with this Code of Conduct may result in warnings, suspension, or termination of membership without refund of the current month.
- a) First violation will be documented via email or text message from club management to the violating member.
- b) Second violation will be documented via email or text message from club management to the violating member.
- c) Colacurcio Wellness LLC (DBA: Vitality Modern Wellness) reserves the right to terminate or suspend your membership if one or two violations of this code of conduct policy has been violated by you and or documented as per 24. a) and 24. b) of this document
- Reporting: Members and staff should report any violations to management.
